S3 recording fields

Purchaser and supply fields, jurisdiction-dependent requirements and outcome options.

Fields shown in Record S3 Encounter and Edit Encounter. Required below means required by the form; this is a reference to Light's behaviour, not a statement of legal recording requirements.

Purchaser lookup

Purchaser details are entered before a new encounter. They cannot be changed in Edit Encounter.

Field	Form requirement	Details
Identification Type	Required	Available types depend on the site's jurisdiction. Types outside that selection appear disabled.
Identification Number	Required	Used with the identification type for the initial lookup. The form checks its format for the selected type.
Given Name	Required	May be prefilled from an earlier encounter at the current site.
Surname	Required	May be prefilled from an earlier encounter at the current site.
Address	Required	Search and select an address, or use Enter manually. The lookup needs a street address, suburb and postcode.
Address Line 1, Suburb, Postcode	Required in manual entry	The separate fields used when entering an address manually.
Address Line 2	Optional	Additional address details.
State	Available in manual entry	The purchaser's address state. The site's jurisdiction controls the supply form's requirements.

Supply and outcome fields

Field	Form requirement	Details
Pharmacist Name	Varies by site jurisdiction	Select a named site user or enter a name.
Product Name	Required	Select a search result or create a custom product. A typed name alone cannot be saved.
Script Identifier	Varies by site jurisdiction	Text field for a script number.
Date of Supply	Required	Prefilled for a new encounter. Check the date before saving.
Directions for Use	Varies by site jurisdiction	Text field for directions.
Supply Outcome	Required	Defaults to Supplied for a new encounter. Options are listed below.
Notes	Optional	Additional details about the encounter, including a condition or refusal reason.

Supply Outcome options are Supplied, Supplied with Condition, Denied, Abandoned and Returned. The selected outcome is saved with the encounter. Notes remains optional for every outcome. Product Name and Date of Supply are required even when the outcome is Denied or Abandoned.

Fields by site jurisdiction

The form uses the jurisdiction configured for the site's physical address. Fields marked Optional are available through Show additional fields. Required fields appear without opening that option.

Consumer fields

The consumer is the person using the product. Purchaser is the consumer is selected by default for a new encounter. Clearing it reveals Consumer Details.

The requirements below apply whenever Purchaser is the consumer is cleared. At sites where separate consumer details are optional, use Show additional fields to reveal the option. Once selected, the consumer's name and address are required.

There are no consumer identification fields in this form.
